Our Story

The increasing prevalence of chronic kidney disease in the state of Hawaii is a growing public health concern. The treatment options for End Stage Renal Disease (ESRD) patients include in-facility hemodialysis, transplantation, or home dialysis.  Home dialysis options are proved to have better survival rate and outcomes than in-facility conventional hemodialysis.  Many articles show that patients doing more frequent home hemodialysis will have more energy, better quality of life, and have less hospitalizations.  

In the past, ESRD patients were unable to access home hemodialysis options if they did not have a care partner. Home health agencies and dialysis companies cannot offer any dialysis care partners to assist home dialysis clients. Foster/care homes also will not accept dialysis patients due to the HD transportation issues or their special needs for home dialysis.
In response to these challenges, we are committed to supporting care homes, as well as ESRD patients and their families, by providing safe and high-quality at-home dialysis treatments. We’re here to help you with your home dialysis needs and concerns!

Meet our team

Jane Takebayashi – ANP-C, APRN-RX, CCRN, CNN

Clinical Director

Jane Takebayashi has a nursing background of over 31+ years, 14 of which is a Nurse Practitioner. She earned her nursing degrees at the University of Hawaii at Manoa. Her RN experiences as an RN include, ambulatory outpatient clinics, medical/surgical nursing, acute hemodialysis, and intensive care units. After receiving her masters degree, she returned to the university as a nursing instructor at the University of Hawaii and Hawaii Pacific University.

As a nephrology APRN/Nurse Practitioner, she managed the care of countless end stage renal disease (ESRD) patients by evaluating their labs, prescribing medications and overseeing their overall health and wellbeing. In her new career as a care home administrator/operator, Jane has seen the great need for renal care among fragile seniors who may have difficulty with outpatient dialysis treatments. She is also a pioneer in starting hemodialysis and peritoneal dialysis in her own care homes. Her goal is to provide and support other foster/ care homes and other ESRD individuals, with the ability to do dialysis within their home or facility safely, greatly improving their quality of life.

Silverio Arana – JR, BSN, RN, CNN

Clinical Manager

Silverio (JR) has been practicing in the medical field for 25 years, of which 21 years was in dialysis. He started as a Certified Nurse Assistance, and transitioned to a Certified Hemodialysis Technician, and eventually becoming a Certified Nephrology Registered Nurse. His experiences include all dialysis treatment modalities: acute Hemodialysis, outpatient hemodialysis, home peritoneal dialysis and home hemodialysis.

As a home dialysis RN, JR has trained many patients and families to perform home dialysis themselves. He is also the lead trainer for newly hired dialysis nurses. With his professional skills and knowledge, he provides excellent teaching to his patients and staff to feel confident that their treatments are done safely and correctly.
