Home Dialysis Care Partners of Hawaii (HDCP) supports individuals across Hawaii who are living with end-stage renal disease. We partner with patients and their families to make dialysis at home, including home hemodialysis (HHD) and peritoneal dialysis (PD), safer and more manageable.
